Cant watch on TV from iPhone

Whats going on? My iPhone connected to TV works for everything except on this app. I get audio but no video. My iPhone image is mirrored on the TV until I start trying to view a DIRECTV show; thats when the video goes away.

The combo of these 2 things, AT&T wireless and DirecTV, is why I got off cable.

Plus the app is laid out illogically. Its hard to do the same thing 2 times in a row. Too much detail to enter here but come on, give me a decision tree that is complete and comprehensible. I dont think these programmers ever actually use the app.
